Dunno. It's a tough call, but I just really liked the 2nd form.

3rd form, all the way. Its the only true Terror of Death form.
I want those claws! I grew to like the shoulder thingies, too, after a while.
(although, his hips looked like he should bear children, or he was a jester or something. And his feet were weird o_O).

There is no meaning in our world
And no meaning is us who live there
We, meaningless, think of the world
Although knowing that there is no meaning there
Is even meaningless itself

Heh...
1st Form showed how much he was a newb in Roots and how weak he felt after facing Azure Kite. Totally understandable, considering Japan's new trend on making their heroes look less manly then usual (See: Vaan, Samurai Legend Musashi).
2nd Form does its job. It was a sight for sore eyes, and a better upgrade from 1st Form. Very Flash. Very Cool.
3rd Form I'm a little mixed with. I was exited when Haseo job extended into it, like "Alright! He got back what he lost!" and how the gameplay somewhat changes when he gets to equip Scythes. However, I was rather disappointed with the actual in-game model. That is, the thighs. They look unnatural in-game and a bit discolored. While the rest of his armor has a shade of black, his thighs look grey. Oh well, I enjoy the scythe. =D
Xth Form is my favorite. Haseo no longer has that black and red color scheme, going for a general white, something I haven't seen a hero done in ages. I just wonder how the dual guns affect the gameplay. See, Haseo's Adept Rogue forms have him be a frontline fighter most of the time, but wit h Dual guns, this changes to having an option to attack from afar. I will await Vol3 to see how much do the guns add.

Also, btw, the guns are useful from shooting far away but the farther you are, the weaker your bullets are. You can move while you are shooting (sort of) and you have a dash feature added to your character. It's basically awesome for avoiding in-your-face combat. The downside is you need a VERY high combo for a Rengeki.
*Note* I surprised how many different opinions there are.
