Where to you get the first two again? i don't remember getting them anywhere (were they from..)

are you talking about the .hack//sign characters and the key items they give you? if so you go to Hidden Forbidden Holy-Ground.

Plus, they are M:wavemaster, Twin Blade, Heavy Axeman, Heavy Blade, Long Arm, Blade Master.
(Im only certain that the first three are in order, cuz those are the only ones i have right now. when i have all 6, i'll update.)
