Need to know which episodes of .hack//SIGN had the flashback-like scenes with the real-life Tsukasa in the hospital. I'm looking for the episode when the father pulled the plug specifically. Please help. Your assistance will be much appreciated.

Episode 12: Entanglement.

It has the flashback of Tsukasa's father talking to a doctor about her condition. Near the end of the episode when Tsukasa is tearing the vines Morganna has placed on Aura he collapses and grasps his chest and there's a real life glimpse of An (Tsukasa's real name, or was it Ana? Oh well) having her father pull the plug.

 no actually stukasa father wanted a boy and well stukas its a girl. his father forced her to wear boys clothes act like a boy and be a boy. and somehow he hated his own daughter. glad other parents arent like that.

Tsukasa was also deprived from 'female products' as well.
In one of the flash backs, Tsukasa's real self is at a table with a woman, and on the table is a brand new bra and deodorant (?). We can assume it's for her because her father comes in and slaps her, and they fall to the ground.

Shes in Holding with a Police officer at Shimokitazawa, she was stealing feminine products.

Specifically, a bra and some tampons.

Question about Tsukasa and her mother. Is it ever revealed "HOW" she died, or is that up for discussion?

There's nothing answered about it. I would assume car crash, just as a throwback to Emma Weilant.
